The Cambria County nursing home is in trouble with the state health department for the second time this year.Commissioner P.J. Stevens said he was surprised by the announcement on Wednesday, when he learned the facility is once again on a provisional license, which means the state health department will be keeping a close eye on the staff to ensure the safety of the residents.The license change comes after violations involving a woman who fell from a lift at the facility and died several days later.The provisional license will last for a period of six months, but Stevens said he is hopeful they will have the facility operating to state standards by October.The staff at Laurel Crest has dealt with several violations with the health department involving the patient lifts over the past several years.Stevens said they are working to make changes to the facility so they can operate without concern from the state.In addition to the license change, the county was also fined $9,500.
